Comprehensive Assessment & Treatment Planning
Clinical care begins with a thorough assessment to understand each person’s mental health needs, strengths, goals, and support systems.
- Initial Assessment: Residents meet with a therapist for 1–3 sessions to complete a Comprehensive Clinical Assessment.
- Individualized Treatment Plan:
- Developed collaboratively with the resident
- Centered around personal goals and stated in the resident’s own words
- Shapes the therapeutic services and supports provided
- Supportive Involvement: Family or trusted supports may be included, if the resident chooses.
- Ongoing Review: The plan is reviewed and updated regularly to track progress and adapt to evolving goals.

Psychiatry & Medication Management
We provide thoughtful, individualized psychiatric care to support each resident’s well-being:
- Psychiatric Intake:
- Conducted shortly after admission
- Reviews medical and mental health history
- Evaluates medication needs
- Ongoing Medication Management:
- Appointment frequency is tailored to each resident
- Decisions are made collaboratively and reviewed regularly
- Focused on safety, effectiveness, and individual choice
Wrap-Around Services
Wrap-Around is a 6-week, post-discharge extension of support. Individuals may choose up to two members of their Averte clinical team to provide weekly check-ins and guidance as they move forward.
Services include:
- 6 weeks of support after discharge
- Up to 2 hours per week with up to two clinical providers (such as a primary clinician, skills specialist, or vocational counselor)
- Sessions offered by phone or telehealth, with in-person sessions available when someone remains in the local area and has their own transportation
- Support focused on transition and follow-through, including:
- Reinforcing skills and strategies learned in treatment
- Navigating community resources and next-step services
- Identifying solutions to challenges as they arise
- Strengthening goal-setting and accountability
- Collaboration with new providers to support a “warm hand-off” and share relevant information
- Safety planning to support ongoing stability
Who It’s For
Wrap-Around Services may be a good fit for individuals who want:
- Extra support during the first several weeks after leaving residential treatment
- Help translating treatment progress into real-life routines and next steps
- Continued connection with trusted members of their care team as they establish new supports
What Wrap-Around Services Are Not
To set clear expectations, Wrap-Around Services do not include:
- Psychotherapy
- 24/7 access to staff
- Crisis intervention services
- Transportation support
