Whether recently diagnosed, leaving the hospital, or just needing a little extra support to be successful in the world, we have a program to fit your needs. Averte bridges the gap between the custodial care of an inpatient stay and the demands of independent living.
At Averte, we believe there is no “one size fits all” approach to mental health treatment. Just as we are all unique and individuals, so must be our approach to your care. Our program is designed to allow for a large degree of customization within an existing framework. Upon admission, we conduct a thorough assessment used to plan a collaborative treatment plan, integrating clinical and residential care.
It’s important we understand the goals, treatment preferences and expectations of each individual as well as their supports. We’ve provided a framework below for different treatment approaches and timelines, but it is important to note that these are fluid and goals can change throughout treatment. It’s important that all parties communicate about questions, concerns, and progress through the program.
Each of our residents has their own personal space, be it an individual bedroom, on campus condo, apartment, or cottage, or a home in the community, and placement may be fluid during one’s time in the program. Regardless of the treatment approach, location or timeline, the following services are available as part of one’s treatment at Averte:
Are you looking for help making real change and establishing long-lasting habits that will help you be successful? Perhaps you’ve been recently diagnosed with a mental health condition, are looking for the next step after a hospitalization, or seeking to gain skills to allow for increased independence and deeper, more meaningful relationships.
Averte’s Transitional Program is designed to meet the needs of individuals who are interested in making a paced transition from residential care to independent living, whether an independent lifestyle near natural supports or in Averte’s Independent Living Program.
In this program, residents stay at one of Averte’s residential homes while receiving 24-hour access to staff support, comprehensive clinical care, enjoying opportunities to frequently access the community, seeking employment, and more.
Many individuals suffering from complex mental health diagnoses benefit from being able to set their own pace in treatment. Averte’s Comprehensive Program provides an opportunity for individuals needing a longer window of services to receive quality treatment at a slower pace. The Comprehensive Program is anticipated to be between 9 – 18 months, and includes all services included in the transitional program.
Some individuals come to Averte looking for a community-based, supportive setting where they can establish a life for themselves. Are you seeking a place where you can find stability with your mental health, have the needed support as you navigate life’s ups and downs, and enjoy both the benefits of professional mental health treatment and the social aspects of living in a community?
Averte’s Extended Program is designed to meet the needs of individuals seeking long-term support or interested in “aging in place.” This program has no maximum stay to allow the individual to remain in the program as long as he/she requires. Although this program continues to offer therapy, the focus is on physical wellness and quality of life. All activities will continue to be encouraged and potential movement within the program explored.
We believe in helping individuals reach their maximum potential and lead fulfilling lives in line with personal values. Often, clients come to Averte with a goal of living independently in their own homes, finding competitive employment, and managing their lives and mental health on their own with natural, community-based supports.
Averte’s Independent Living Program provides clients with a safety net and support system as they practice independent living skills and gain confidence in their own home or apartment in the greater community. For some, this program may be long-term; for many, it is a stepping stone. While receiving specialized support for their next steps to independence, participants in this program are still considered members of Averte’s residential community; they can visit our houses for meals, support services, activities, therapy groups, socialization and continued clinical care. Direct admits are accepted in the ILP if all parties agree it is a safe option.
Clients in the Independent Living Program have the option to lease or buy their own living space near one of our existing residential programs or move into an Averte-owned apartment or condo while in the program.
Averte’s Respite Program is designed to meet the needs of an individual requiring a short reprieve from their current living situation. This program has a maximum length of 30 days and is less clinically intensive. The goal during this time is safety and stabilization, and services provided may vary depending on the individual’s needs. Stays may be extended into our Transitional Program if appropriate and desired.
