Averte’s Comprehensive Program provides an opportunity for individuals needing a longer window of services to receive quality treatment at a slower pace. The Comprehensive Program is anticipated to be between 9 – 18 months that will be evaluated quarterly.

Extended Program

Averte’s Extended Program is designed to meet the needs of an individual with open-ended care. This program has no maximum stay to allow the individual to remain in the program as long as he/she requires. Although this program continues to offer therapy, the focus is on physical wellness and quality of life. All activities will continue to be encouraged and potential movement within the program explored.

Independent Living Program

Averte’s Independent Living Program provides a bridge for those transitioning out of residential care and into the greater community. While receiving specialized support for their next steps to independence, participants in this program are still considered members of Averte’s residential community; they can visit our houses for meals, support services, activities, groups and socialization.

Respite Program

Averte’s Respite Program is designed to meet the needs of an individual requiring a short reprieve from their current situation. This program has a minimum of 14 days with a maximum of 45 days. Respite stays may be admitted to our Transitional Program if appropriate. Respite stays are dependent upon availability.

Transitional Program

3 - 9 months

Averte’s transitional program is targeted to be between 3 – 9 months long with a transition to our Independent Living Program or a return to an independent lifestyle near family.

  • Weekly Psychiatry Appointments as needed for medication optimization
  • Expected to attend all appropriate clinical groups and participate in volunteer or work program
  • Will receive individual therapy appointments weekly.
  • Family involvement as recommended by clinical team
  • Will participate in no less that 7 community activities per week and physical wellness program
  • Will receive weekly 1/1 vocational support and groups
  • Maintain at least 30 hours of structured activity per week

Comprehensive Program

9 - 18 months

Averte’s Comprehensive Program provides an opportunity for individuals needing a longer window of services to receive quality treatment at a slower pace. The Comprehensive Program is anticipated to be between 9 – 18 months that will be evaluated quarterly.

  • Psychiatry appointments at least monthly for medication optimization
  • Weekly individual therapy
  • Expected to attend 5 – 7 groups per week
  • Will participate in at least 5 community activities per week
  • Will participate in a physical wellness program
  • Maintain at least 20 hours of structured activity per week
  • will receive weekly 1/1 vocational support and vocational group

Extended Program

Open-ended

Averte’s Extended Program is designed to meet the needs of an individual with open-ended care. This program has no maximum stay to allow the individual to remain in the program as long as he/she requires. Although this program continues to offer therapy, the focus is on physical wellness and quality of life. All activities will continue to be encouraged and potential movement within the program explored.

  • Bi-monthly psychiatry appointments as needed for medication optimization
  • May participate in all community activities and groups
  • Option to be in bi-weekly individual therapy
  • Option to utilize supportive counselling weekly
  • Option to participate in volunteer and work program
  • Option to join the physical wellness program
  • Maintain at least 15 hours of structured activity per week

Independent Living Program

Open-ended

Averte’s Independent Living Program provides a bridge for those transitioning out of residential care and into the greater community. While receiving specialized support for their next steps to independence, participants in this program are still considered members of Averte’s residential community; they can visit our houses for meals, support services, activities, groups and socialization.

  • Available housing on and off-site (not included in fee)
  • Four apartments on Averte main campus
  • Program supports apartments in both Bradford, VT and Hanover, NH areas
  • Option to rent or buy own housing in community
  • Life skills supports including cooking, financial and home-care skills
  • Continued access to all residential programming
  • Continued care from the same clinical treatment team
  • Individual and group vocational support

Respite Program

14 - 45 days

Averte’s Respite Program is designed to meet the needs of an individual requiring a short reprieve from their current situation. This program has a minimum of 14 days with a maximum of 45 days. Respite stays may be admitted to our Transitional Program if appropriate. Respite stays are dependent upon availability.

  • Expected to attended recommended groups and encouraged to participate in all community and volunteer activities
  • Option to utilize supportive counselling weekly
  • Option to join the physical wellness program
  • Maintain at least 20 hours of structure activity per week
